Step 3: What do I do once my application is fully completed?
-
The student will bring the completed application to Central Office (70 Low Street, Newburyport, MA 01950) AS WELL AS one form of official identification, such as a birth certificate, passport, or a driver's license/permit.
-
Central Office will make a copy of the identification and create the permit. The student will then sign the permit in person.
-
The application, copy of identification, and permit will be kept on file. A permit is only valid for two years.
