- 9-12: Newburyport High School, Jessica LeBlanc and Lauren Gee (Please email. No appointment is needed.)
NPS has an English Learner Family Liaison who can assist families who do not speak English as a first language. Please let the school receptionist know if you need translation support.
You Must Complete the Following:
1) Required Documents
- Student’s birth certificate

Student’s most recent health record including lead test, immunization and physical (for more details see Health Entry Requirements)

For enrolling in grades 10-12 only: a copy of student's high school transcript as well as a list of current courses

Proof of Newburyport Residency. You must bring one document from each column:

2) Required Forms
There are 6 required Forms. Please print and complete all 6 forms and bring to your enrollment appointment.
(1)Newburyport Public Schools Enrollment Form
(2)Confidential Health Form*
(3)Certification of Address Form
(4)Language Surveys
(5)Medicaid Consent Form
*If your student is entering the district mid-year, please fill out our Annual Student Health Form in addition to the Confidential Health form.
(6) Student Record Release Forms
This form allows the school your student is attending to send records to Newburyport. Select the school and language. Print, complete, and return with your enrollment forms.
