Physicals and Screenings

Massachusetts School Health law requires screenings for vision, height and weight, hearing and posture. Comprehensive Physical Exams are required on entrance to school and in grades K, 4, and 7. Below is a schedule of regularly performed screenings as well as schedule for state required physical examinations.

NOTE: You can request a vision or hearing screening at any time if you suspect that your child may have a problem.

What Does a Referral Mean?

If you receive a referral form after any school screening, it simply means that the screening detected a possible problem and further evaluation is recommended. The form is to be completed by your primary care provider, dentist or eye doctor and returned to the school nurse. As always, if you have any questions, contact the school nurse.

 

 

A note about physical exams: It is recommended that this exam be done by your primary care provider who is more familiar with your child's medical history. You can call your school nurse for the form needed to report a private physical exam or click here for forms posted online.

Athletic Physical Exams (grades 9-12) Students must have a current (within 13 months) physical on file prior to being allowed to participate in high school athletic activities.
