Registration

• All Spring sports start on MONDAY, MARCH 19th (check x2 under NHS Athletic Department for practice times)

In order to register your son/daughter for a Spring sport you MUST:

Visit Pat McBride in the main office ANY weekday from 7am-noon or 1pm-3pm March 5-9 or March 12-16.

OR…

Visit Pat McBride and A.D. John Daileanes in the main office on Monday, March 5th any time between 6pm-8:00pm.

What you will need to do at registration:

• Complete the Registration Packet (the registration packet cane also be downloaded from X2 in teh NHS Athletics Department page under the Forms section
• Provide the office with an updated physical (dated February 19th, 2011 or more recent).
• Complete the state-required Pre-Participation Head injury Form and sign off that you've completed the online concussion course.
• Write a check for the user fee (see the tiered structure below).

Users Fee Structure

• Grade 9, 10 baseball, softball, lacrosse $275
• Grade 11, 12 baseball, softball, lacrosse $340
• Grade 9, 10 track, tennis $200
• Grade 11, 12 track, tennis $265

The Consent Form, Physical, Concussion Certificate and Users Fee are all due BEFORE THE FIRST PRACTICE OR THE STUDENT IS NOT ELIGIBLE TO PARTICIPATE!!

NFHS Online Concussion Course Instructions

To access the online NFHS Concussion Course follow these directions:

1. Go to http://www.nfhslearn.com
2. Click on "Register" and complete the basic information
3. Click "Submit Registration"
4. Click on the course, "Concussion in Sports - What You Need to Know"
5. Click "Order Now"
6. Select "Order Course for Myself"
7. Choose "Massachusetts" and Hit "Save"
8. Select "Complete Purchase" ($0.00)
9. Take Course
10. After completing the course, go to "My Homepage/Courses"
11. Scroll down the page to where it says, "Completed Courses"
12. You can print the Concussion Certificate from the "Completed Courses" section
